BAYADA Reports 75% Reduction in Major Fall Injuries Through AI Care Model

A 75% reduction in major injuries from falls suggests that predictive technology, when paired with nurse-led coordination, fundamentally changes home health outcomes. By analyzing over 40 client data points to identify high-risk windows, BAYADA Home Health Care is shifting the industry toward targeted, rather than just increased, care hours.

Since deploying its Enhanced Quality of Care model nearly a year ago, the nonprofit provider has observed a decline of roughly 58 major fall-related injuries annually among its 8,000 private-pay clients. The system functions by flagging routine high-risk periods—such as morning transitions or bathing—allowing clinical managers to align staff presence with the times patients are most vulnerable. This approach avoids the common industry pressure to simply add more hours, focusing instead on the strategic placement of care.

For patients like Debra Chapman, who experienced 26 falls in the six months preceding the program, the impact has been transformative. With a dedicated aide now present during high-risk windows, Chapman has recorded only two falls over the past year, both resulting in no injuries. This shift has allowed her to return to social activities and travel, highlighting the role of consistent oversight in maintaining independence.

While the industry currently lacks standardized benchmarks for fall-related harm, BAYADA is utilizing its own historical performance to measure progress. The organization is now working with partners, including AlayaCare, to refine these predictive tools and is exploring ways to adapt the model for Medicaid populations. Although these initial findings are internal and await independent validation, they signal a growing movement to quantify and improve safety outcomes in home-based care.
