The app functions as a centralized hub for navigating complex care transitions, allowing clinicians to submit referrals with minimal taps on their mobile devices. By integrating interactive eligibility assessments alongside expert-led educational content, the company seeks to address the issue of late-stage hospice enrollment, where patients often miss out on the full benefits of palliative support due to communication delays.
Patrick Hale, chief information officer at VITAS, noted that the software is engineered specifically to simplify workflows for busy medical professionals. The application is now available for download on both Apple and Android devices, supporting the organization's network of 60 service areas across 15 states and the District of Columbia.
